Community Follow-up Program
The Community Follow-up Program is a Medicaid-reimbursed,
comprehensive case management program for individuals with HIV/AIDS.
Through the use of its staff, the program addresses the complex issues
associated with HIV/AIDS by assisting the client with the coordination
of services and advocacy to ensure appropriate service delivery. The
ultimate goal of the program is to assist the client in becoming empowered
to effectively advocate on their own behalf.
